All participants in Athletics, Marching Band, Cheerleading, Drill Team, and ROTC are required to have a physical on file with the Athletic Training Department

Return the completed Medical History & Exam Form to the Sports Medicine Staff at College Park High School
- Through the front desk (OK but not the best way to guarantee form submission)
- Directly to a staff Athletic Trainer (BEST)
- To your coach (Knox Jr High ONLY) (RISKY) -> CP athletes never submit medical documents to coaches
